BPC-157: Research Summary

Mechanism
BPC-157 is a synthetic pentadecapeptide (15 amino acids) derived from a partial sequence identified in human gastric juice, from which the "body protection compound" name is drawn. It is not a native full-length protein and no endogenous BPC-157 receptor has been definitively cloned. Reported mechanisms in the pre-clinical literature include modulation of the nitric oxide (NO) system, upregulation of growth-hormone-receptor expression in tendon fibroblasts, promotion of VEGFR2 signaling and angiogenesis, and activity on dopamine, serotonin, and GABA pathways in central and enteric nervous system models.
Published trials
The evidence base is overwhelmingly pre-clinical (rodent), with the majority of primary literature originating from a single research group in Zagreb and collaborators. Reported findings span tendon-to-bone healing (Achilles transection models), muscle crush and detachment injury, corneal wound healing, gastrointestinal ulcer models (including NSAID-, alcohol-, and stress-induced), fistula closure, inflammatory bowel disease models, and vascular reperfusion following ligation.
Human clinical data is sparse. A small early Phase 1/2 trial in inflammatory bowel disease was conducted; published human safety or efficacy data at the scale of a modern RCT is not available. Independent replication of the pre-clinical work outside the original research group is limited, and this is a recognized gap in the literature.
Safety profile
Rodent toxicology data reports no LD50 within tested dose ranges and no observed target-organ toxicity across acute and sub-chronic exposures. Published human safety data is limited to small early-phase studies and anecdotal clinical reports; no long-term controlled human safety data exists. BPC-157 is a WADA-prohibited substance in and out of competition, which is a separate regulatory rather than toxicity consideration. It is not approved for human therapeutic use by Health Canada, the FDA, or the EMA.
How it compares in its class
BPC-157 sits in a small class of "regenerative peptides" often grouped with TB-500 (thymosin beta-4 fragment), KPV, and larger growth factors like GHK- Cu. Within this class, BPC-157 has the deepest pre-clinical literature on gastrointestinal indications and the most-cited tendon and gut-barrier pre-clinical models, but the shallowest human clinical data. Comparisons across these compounds are pre-clinical only and should not be read as clinical superiority claims.
