Semaglutide: Research Summary

Mechanism

Semaglutide is a 31-amino-acid analog of native glucagon-like peptide-1 (GLP-1) with two substitutions that resist dipeptidyl peptidase-4 degradation and a C18 fatty di-acid moiety that binds albumin. The net effect is a half-life of roughly one week, supporting once-weekly subcutaneous dosing (and, in an alternative oral tablet formulation co-formulated with SNAC, a daily oral option). It is a selective GLP-1 receptor agonist: no meaningful activity at GIP or glucagon receptors. Downstream effects include glucose- dependent insulin secretion, glucagon suppression, delayed gastric emptying, and central appetite modulation via hypothalamic and hindbrain circuits.

Published trials

The SUSTAIN program (10 Phase 3 trials in type 2 diabetes) established semaglutide's glycemic and weight profile: HbA1c reductions of roughly 1.4–1.8 percentage points at 1.0 mg weekly, with 4–6 kg mean weight loss. SUSTAIN-6 was the dedicated cardiovascular outcomes trial and reported a statistically significant reduction in major adverse cardiovascular events versus placebo in T2D participants at elevated cardiovascular risk.

The STEP program extended semaglutide into obesity indications. STEP-1 reported roughly 15% mean body weight reduction over 68 weeks at 2.4 mg weekly in participants without diabetes. STEP-2 through STEP-8 covered variations in population (T2D, adolescents, background lifestyle intervention, race/ethnicity subgroups) and delivered a consistent 10–17% weight-loss signal. SELECT reported cardiovascular outcomes in participants with overweight or obesity and pre-existing cardiovascular disease (without diabetes) and met its primary MACE endpoint. FLOW reported reduced kidney-outcome events in T2D with chronic kidney disease.

Safety profile

Gastrointestinal adverse events — nausea, vomiting, diarrhea, constipation — dominate the tolerability profile and cluster during dose escalation. Published safety literature discusses risk of pancreatitis, gallbladder disease, acute kidney injury via dehydration, and the rodent-derived medullary thyroid carcinoma signal (contraindicated in personal or family history of MTC or MEN2). Diabetic retinopathy complications were noted in SUSTAIN-6, generally in participants with pre-existing retinopathy experiencing rapid glucose lowering.

How it compares in its class

Semaglutide is the reference GLP-1 monoagonist for both T2D and obesity, and the compound with the most extensive published cardiovascular and renal outcomes evidence. In head-to-head trials it has outperformed earlier GLP-1 agonists (liraglutide, dulaglutide, exenatide) on HbA1c and weight endpoints. Against dual (tirzepatide) and triple (retatrutide) incretins, semaglutide's weight-loss magnitude is smaller in reported Phase 3 comparisons, but its long-term outcomes and safety data set remains the deepest in the class.
