SS-31 (Elamipretide): Research Summary

Mechanism

SS-31 (elamipretide, MTP-131, Bendavia) is a synthetic Szeto-Schiller tetrapeptide (D-Arg-dimethylTyr-Lys-Phe-NH2) that selectively partitions into the inner mitochondrial membrane and binds cardiolipin. Cardiolipin is the signature phospholipid of the inner mitochondrial membrane and is required for optimal organization of the electron transport chain supercomplexes. SS-31 binding stabilizes cardiolipin, protects it from peroxidation, and preserves cristae architecture and ATP synthase efficiency. Unlike most peptides, SS-31 penetrates cell membranes and localizes intracellularly without a specific receptor.

Published trials

Pre-clinical work spans ischemia-reperfusion injury (heart, kidney, brain), heart failure, Alzheimer's disease models, macular degeneration, and skeletal-muscle aging. In aged mouse skeletal muscle, short-course SS-31 has been reported to acutely restore ATP-generating capacity, improve fatigue resistance, and normalize redox status. Clinically, the compound has been evaluated in randomized trials for primary mitochondrial myopathy (MMPOWER-3), Barth syndrome (TAZPOWER), and geographic-atrophy dry AMD (ReCLAIM). Results have been mixed: some trials met secondary functional endpoints while missing primary endpoints, and the compound has not, as of writing, received a first regulatory approval.

Trials in heart-failure with preserved ejection fraction and in dry AMD have continued into subsequent readouts, with some positive functional signals reported in extension studies. The full regulatory trajectory remains open.

Safety profile

Published trials at the doses studied (typically 40 mg subcutaneous daily in the mitochondrial myopathy and Barth trials) reported a favorable tolerability profile. The most common adverse events were injection-site reactions. Serious adverse events were infrequent and generally not attributed to study drug in the published reports. Long-term controlled safety data at hard-endpoint scale is not yet available.

How it compares in its class

SS-31 is the most-studied member of a small class of mitochondria-targeted therapeutics that includes the antioxidant MitoQ (a triphenylphosphonium- tethered ubiquinone) and other cardiolipin-stabilizing agents in earlier development. Its mechanistic niche — direct cardiolipin binding and cristae stabilization — is distinct from MOTS-c (transcriptional/AMPK) or NAD+ (coenzyme replenishment) approaches to mitochondrial dysfunction, and the three are sometimes discussed as complementary strategies rather than competing ones.
